It should take about 10-20 minutes to fill out. Michael at the office will receive this form directly, and will add it to Equipped to Serve 2024-2025 to be published on July 1st. While that date seems far off, it takes a long, long time to compile all of these forms into a cohesive booklet. With that said, please be aware:

  • The deadline to submit this form is Friday, May 3rd. 
  • Forms will NOT be accepted after May 3rd. 
  • Forms must be submitted on time in order for your ministry to appear on our list of ministries in Equipped to Serve.
